The Patient Portal provides an electronic method for you to be involved in your health care.  You can request appointments, review portions of your health information, send secure messages to your physician, request prescription refills, review clinical notes and much more.

In addition to our patient portal, your health information can be accessed through an application such as those used on a smartphone. Depending on the chosen application, you may be able to access information such as your medications, allergies, or lab results. To begin this process please reach out to our Patient Portal Support.

Your log on ID and password can be reset by using the links on the Patient Portal logon page.  This will also unlock a locked Patient Portal. When these requests are made the information entered must match your Patient Portal account. If you need assistance, please contact Patient Portal Support.

Don’t have a Patient Portal account?

Please note, you must be 18 years and older or emancipated to sign up for a Patient Portal.

There are two ways to sign up for a Patient Portal account:

  1. Visit a St. Peter’s Health location and fill out a Patient Portal Registration form. You must sign and complete ID verification in person
  2. Sign up for a Patient Portal online. IMPORTANT NOTE: An email must reside on your medical record in order for this to succeed. If you are using your Social Security Number instead of your Medical Record Number, your Social Security Number must also be present on your medical record.

Medical Record Number

Keeping your health care information private and secure is of the utmost importance.  Medical Record Numbers will not be provided by email or over the phone.  Your Medical Record Number starts with an “M” and looks like M000055555.  The entire number (including zeroes) needs to be entered when using it to sign up for Patient Portal on-line.  You can find your Medical Record Number on:

  • Your billing statement
  • Your hospital discharge papers
  • Your hospital wristband
  • Your clinic visit summary

Accessing other patient’s health care information

If you are an adult (Proxy) wanting electronic access to another patient's health care information through a Patient Portal, a Patient Portal Proxy Registration must be filled out and submitted.

Registration Forms

An individual Patient Portal Registration Form is a legal document completed by by an adult patient giving St. Peter's Health permission to create a Patient Portal.

A Patient Portal Proxy Registration form is a legal document completed by an adult patient giving a designated adult (Proxy) permission to access their health care information electronically. The proxy agrees to adhere to rules outlined on the form.

If you are a St. Peter’s Health patient wanting electronic access to another adult’s health care information, you must have your own Patient Portal prior to filling out a Patient Portal Proxy Registration form.

If you are not a St. Peter’s Health patient but want electronic access to another adult’s health care information, you must complete the Patient Portal Proxy Registration form. A non-St. Peter's Health portal will be created for you. 

Forms below can be downloaded and filled out but they must be signed in the presence of a St. Peter’s Health employee or a notary who has validated your identity. Notarized forms must be hand delivered to a St. Peter’s Health location or faxed to Patient Portal Support.
