In order to better assist you with your communication or marketing needs, the St. Peter's Health PR/Marketing department has created a set of online marketing request forms for:

  • Marketing/Graphic design requests (new or need revisions)
    • Brochures
    • Posters
    • Advertisements, etc)
  • New/revised medical forms
  • Graphics/printing
    • Reprints
    • business cards,
    • stationary, etc
  • Website/Intranet updates

To make a request, please complete and submit the appropriate form linked below.

Please note: Requests are reviewed on a weekly basis. Once we have received, reviewed and/or completed your request, a member of the team will contact you. Standard turnaround times for commonly requested items noted on each form. Actual timelines vary depending on actual scope of work, i.e. content supplied or needing to be created, utilization of outside vendors, feedback deadlines, etc. Requests made without proper notice for standard turnaround times may not be able to be accommodated.

Graphics/Print requests form

Complete this form if you need reprints, business cards, stationary, etc. Please note:

  • For items with no changes, expect 1-2 weeks for in-house items or 2-4 weeks for outsourced items
  • For items with revisions, expect 2-4 weeks for items printed in-house depending on scope of changes or 6-12 weeks for major revisions printed by outside vendor depending on scope of changes

Complete this form if you'd like to request a new marketing or graphic design item (brochures, posters, advertisements, etc.) or if you have a marketing item that needs revisions. Please note that standard turnaround times vary for each item (2 weeks to several months). We may not be able to accommodate your request if it is made without adequate notice.
