Population Health - Behavioral Health Professional

The Behavioral Health Professional (BHP) is an integral member of the primary care team who works to identify, triage and support patients with behavioral health needs in the primary care setting. The BHP specializes in brief, evidence-based, solution-focused assessments and interventions geared toward functional restoration and empowerment. The BHP typically sees patients for 15 to 30 minute appointments and care may address chronic health conditions (e.g. diabetes, obesity and chronic pain), mental health and substance use conditions, health behaviors, life stressors and crises, stress-related physical symptoms and ineffective patterns of health care utilization. Must adapt interventions to the primary care work flow and pace, including rapid assessment, brief treatment and a high daily volume of contact. Collaborate with an amazing interdisciplinary team dedicated to their patients and to each other. As a Hospice Social Worker at St. Peter’s Health, you’ll offer patient and family-centered care to help provide quality end-of-life care in the home. You’ll help develop care plans, along with offering support, education and resources to patients and their families. This position offers great opportunity for career growth, continuing education opportunities and supervision within the Social Work team. A Master of Social Work from a school accredited by the Council on Social Work Education is required. Licensure in the State of Montana Licensed Clinical Social work (LCSW) is optional. Salary and benefits are highly competitive and dependent upon experience and qualifications.
