U.S. Forest Service Lincoln Screenings

Wecome to the sign up page for the U.S. Forest Service Wellness Screenings!  This screening will take place on Thursday, January 31st at 8:30am at the Lincoln location. 

Please sign up using the time slots below and thank you for participating this year!

Wellness Screening Instructions

- No food or drink (aside from water) for 12 hours prior to the screening

- No alcohol for 24 hours prior to the screening

- If you are having any optional tests, please have payment ready at the time of check in

The cost of the basic wellness screening is covered through the U.S. Forest Service. Above are additional tests available at all the St. Peter's Wellness screenings. Payment for additional testing is due at check-in on the day of the screening. We accept cash or check (made out to St. Peter's Hospital).
