Care Management

Care Managers are a team of registered nurses and social workers that are assigned to each patient care area. They work closely with your physician to assist patients and families in dealing with problems brought on by illness and hospitilization.

Care Managers assist with post-hospital care referrals and coordinate services such as rehabilitation, home health care, Hospice, nursing home placement and obtaining medical equipment.

Care Managers can also help you with any questions you have regarding finances such as insurance coverage, Medicaid application, Social Security/disability information or Medicare.
